"I've run into the following situation involving an MPEG-2 video clip and its corresponding Dolby audio file:
During DVDSP preview, the audio falls behind the video at the end of the movie when the narrator is speaking. When I build the disc and play back the presentation in Apple DVD Player 2.7, the synch problem is readily apparent.
Oddly enough, if I build the DVD using the AIF file rather than the AC-3, the audio keeps in synch with the same MPEG-2 clip. When I watch the source DV clip created from Final Cut 3, everything is in synch as well. I've created AC-3 files using A.Pack several times and obtained the same results.
This is the only clip where I'm experiencing this problem. On the same disc I have another narrator clip where everything remains in synch. I'm using DVDSP 1.2 in OS 9.2.2."

To solve this problem try running your MPEG file through
MPEG Append from John Brisbin.

and many authors find that processing their MPEG encodes
with MPEG Append will clean up the video timecode
making it possible for the AC3 track to stay in sync.
